New client referral process:

  • Call 754-200-1328 for a free initial phone consultation.

  • If it sounds like I have the services to help, and you are interested in proceeding to the next step, I will schedule a 90 minute consultation appointment.  There is no commitment to treatment beyond this appointment.  You are encouraged to bring a family member or someone close to you, and the goal of this session is to make sure that the treatment and I are a good fit for you.

  • Prior to the initial consultation appointment, you will be set up with a “patient portal” from my electronic medical record (EMR) called CounSol.  You will need to create an account with your own password (please write these down and save them) and will fill out all necessary demographics and emergency contact information as well as complete the necessary online forms before initial consultation appointment.  

  • Once you've completed the portal, you'll be emailed the intake paperwork that needs to be printed and filled out and returned to Rachel Blogg via her secure email at before meeting for initial appointment with a copy of your photo ID.

  • Due to COVID-19, all appointments at this time are being done via Telehealth on Zoom that is secure and HIPPA compliant  During the consultation appointment Rachel Blogg will meet with you and will include your family member for parts of the session if you wish.  You’ll be asked to share the reasons for seeking therapy, your history of prior treatment, and other pertinent information in order to better understand your situation and needs.  In addition, she will orient you to the types of services she provides.

  • Rachel Whipple Blogg, LCSW, P.A. does not accept insurance and works on a fee for service basis.  If you have out-of-network benefits with your insurance company, please inform her at the initial consultation and the billing statements with all the necessary information will be available via the patient portal so you can submit to your insurance.  She does not submit claims to or receive payments from any insurance companies.  Full payment for services is required when the service is rendered.  She accepts payment by cash, check, or credit card.
